Paying For Child Care
The Child Care Financial Assistance program assists parents in paying for their child care while they are working or attending an approved education/training program. Eligibility and parent co-payment is based on annual income, family size and the number of children receiving child care.
How It Works
- Parent chooses a child care provider
- Parent obtains application
- Parent completes all areas of application pages 1 - 4
- Child care provider completes part of page 5 and all of page 6 of application
- Child care provider signs page 6 and parent signs page 7
- With the application, parents must include two (2) most current, consecutive pay stubs from all jobs and all income eligible adults in the household
- When pay stubs are not available due to new employment, a 4-C verification form may be used
- Students in education/training programs must include a current class schedule with their application
- High School students must include a letter from the school principal or guidance counselor with their application as proof of their enrollment
- Return completed application forms with the above wage information to 4-C via U.S. Postal Service or Fax
